Please find the attached an ‘End of Year Class Profile Sheet’.
Ideal to complete by the current Class Teacher in Term 3 (or at the end of an academic year).
Each part of the profile should be completed with as much detail as possible. Continue onto the next page if necessary.
The completed form will give a clear picture of the whole class, contact information and relevant issues that need to be considered. The details on this sheet can also be used to determine whether a class needs to be reorganised for the next academic year.
